How to Apply

Step 1

Fill up credit card application form.

Application form can be obtained from:

  1. At the end of this page, or
  2. AFFINBANK Branches, or
  3. Cards Business Department

Step 2:

For application processing, please enclosed the following documents

Self Employed Salaried Employee Commision Earner Pensioner
Photocopy of IC/ Passport with Visa Permit valid for at least 1 year, including supplementary applicant (both sides), AND Photocopy of IC/Passport with Visa Permit valid for at least 1 year, including supplementary applicant (both sides),AND Photocopy of IC/ Passport with Visa Permit valid for at least 1 year, including supplementary applicant (both sides), AND Photocopy of IC/ Passport with Visa Permit valid for at least 1 year, including supplementary applicant (both sides), AND
For Sole Proprietor / Partnership:
  • Certificate of Business Registration, AND
  • Latest 1 year B Form / E-Filing with acknowledgement and tax payment receipt,AND
  • Latest 6 months company/personal bank’s statements ORLatest 1 year audited accounts.
Latest 3 months salary slip OR Latest EA Form or BE Form/E-Filing with acknowledgement and tax payment receipt,AND Latest 6 months commission statement OR B/BE Form / E-Filing with acknowledgement and tax payment receipt, AND Latest pension fund statement, AND
For Private Limited Company:
  • Certified true copy of Certificate of Incorporation, Memorandum & Articles of Association (M&A), latest Form 24 & 49, AND
  • Latest 2 years B Form / E-Filing with acknowledgement and tax payment receipt,AND
  • Latest 6 months company/personal bank’s statements ORlatest 2 years audited accounts. 
Latest EPF statementOR Latest 3 months bank statement where the salary is creditedOR Letter from Employer to confirm income (established company/Letter is originally addressed to Bank). Latest EPF statemen OR Latest 6 months bank statement where the commission is credited OR Letter from Employer to confirm income (established company/Letter is originally addressed to Bank). Latest 3 months bank account statement where the pension is credited.
Note:  For Non-Malaysian Fixed deposit (FD) to be pledged for credit card facility
 

Please note that the enclosed documents are non-returnable. For security purpose, please cross all the documents submitted.

Step 3:

Submit the credit card application form along with all relevant documents

Credit Card application must be submitted personally via any of AFFINBANK branches. Personal identification and original income supporting documents must be witnessed by branch personnel.
