How to Apply

Step 1

Fill up the credit card application form which can be  obtained from the following channels:

  1. Click HERE to download the application form;
  2. AFFINBANK branches; or
  3. AFFINBANK Cards Business Department.

Step 2:

Enclosed the following documents

1. Salaried Employee
  • Photocopy of IC (both sides) or passport with valid Visa permit (for non-Malaysian applicant) for both principal and supplementary applicants
  • Latest 3 months salary slip or latest EA form or BE form / e-Filing with acknowledgement and tax payment receipt
  • Latest EPF statement or latest three (3) months bank statement where the salary is credited or letter from employer to confirm income (established company / letter is originally addressed to the Bank)

2. Self Employed
  • Photocopy of IC (both sides) or passport with valid Visa permit (for non-Malaysian applicant) for both principal and supplementary applicants
    Sole Proprietor / Partnership:
  • Certificate of Business Registration
  • Latest  one (1) year B form / e-Filing with acknowledgement and tax payment receipt
  • Latest six (6) months company / personal bank’s statements or latest one (1) year audited accounts
    Private Limited Company
  • Certified true copy of Certificate of Incorporation, Memorandum & Articles of Association (M&A), latest Form 24 & 49
  • Latest two (2) years B form / e-Filing with acknowledgement and tax payment receipt
  • Latest  six (6) months company / personal bank’s statements or latest two (2) year audited accounts

3. Commission Earner
  • Photocopy of IC (both sides) or passport with valid Visa permit (for non-Malaysian applicant) for both principal and supplementary applicants
  • Latest six (6) months commission statement or B form / BE form / e-Filing with acknowledgement and tax payment receipt
  • Latest EPF statement or latest six (6) months bank statement where the commission is credited or letter from employer to confirm income (established company / letter is originally addressed to the Bank)

4. Pensioner
  • Photocopy of IC (both sides) or passport with valid Visa permit (for non-Malaysian applicant) for bothprincipal and supplementary applicants
  • Latest pension fund statement
  • Latest  three (3) months bank account statement where the pension is credited
Fixed Deposit (FD) will be required as collateral for the following applicant category and must be maintained with AFFINBANK for as long as the credit card facility is available.
a. Non-Malaysian applicant; and
b. Applicant who are unable to provide any income evidence or supporting documents.


Note: Please cross out documents submitted for security purposed. All documents are non-returnable.
 

Step 3:

Credit Card application and  supporting documents must be submitted personally by the applicant to any AFFINBANK branches, please bring your original supporting documents for verification.

Note: application via fax, e-mail or directly to AFFINBANK Cards Business Department are not allowed.
