1:
Fill up our credit
card application
Application
forms can be obtained at any AFFINBANK CRC, Service Centres, Card Services Department
or can be downloaded from AFFINBANK's corporate web site.

Step
2:
Enclose relevant documents required to process
the application
-
Photocopy of IC (both sides) or passport, including supplementary
applicants
- Latest Income Tax Return
(J Form) or EA Form
- Latest 2 month's salary slips or letter from the employer confirming
length of service, income & position held
- Latest 3 month's bank statements &
F9, 24, 49/Business Registration Certificate (if self employed)
- Any other income supporting documents

3:
Submit the credit card application form
along with all relevant documents to:
