Fill up our credit card application
Application forms can be obtained at any AFFINBANK Branches and Cards Business Department.

Step 2:

- Copy of IC (both sides), including supplementary applicant
- Copy of latest 2 months salary slip

- Copy of IC (both sides), including supplementary applicant
- Copy of Business Registration / Form 9, 24 and 49
- Copy of latest 3 months Company bank’s statements
- Copy of latest Form B (with Tax payment receipt)

- Copy of passport
- Copy of work permit
- Copy of latest 2 months salary slip
- Fixed deposit to be pledged against credit card facility

Step 3:
Submit the credit card application form along with all
relevant documents
Credit Card application must be submitted personally via any of AFFINBANK branches. Personal identification and original income supporting documents must be witnessed by branch personnel.
Note: Please do not sent via fax or directly to Cards Business Department.
