I authorise Affin Bank Berhad/Affin Islamic Bank Berhad to charge my credit card/debit card(i) for the payment of AFFIN Cards Golf Challenge FY2020, amounting a total of RM388.00 / RM288.00 * only.

* All tournament participants who met the Minimum Spend must pay an entry fee of RM288.00, whereas those who did not meet the Minimum Spend must pay an entry fee of RM388.00.

TERMS AND CONDITIONS
  1. The AFFIN Cards Golf Challenge FY2020 Campaign (the “Campaign”) is organised by Affin Bank Berhad and Affin Islamic Bank Berhad (the “Bank”). The Campaign is subject to the Campaign Terms and Conditions.
  2. ​​An organising committee appointed by the host club shall manage the Campaign staged at the club. ALL DISPUTES shall be handled by the Tournament Committee whose decisions on all matters pertaining to the tournament shall be final.
  3. The Bank or Golf Malaysia shall not be liable for any repercussions (discrepancies, miscalculations of results, prizes or other matters) arising from the failure to submit proper Entry Forms (fully completed).
  4. The Campaign is open to new and existing AFFIN Cardmembers who meet the criteria set out below and in the Campaign terms and conditions.
    • The Eligible Cardmember is required to fulfill the minimum spend criteria of RM6,888  within  the  Spend  Campaign  Period  (the  “Minimum Spend”) from 15 July to 15 October 2020 to qualify and participate in the tournament.
    • The Eligible Cardmember is required to complete the Entry Form for their preferred Qualifying Legs to confirm participation and must reach the Bank no later than 16 October 2020. Please submit e-Entry Form or the Entry Form via e-mail to AFFIN Cards at affincards@affinbank.com.my
    • Allocation of registered entries is based on first-come, first-served basis.
    • All tournament participants who met the Minimum Spend must pay an entry fee of RM288.00, whereas those who did not meet the Minimum Spend must pay an entry fee of RM388.00. The entry fee includes 6% Sales & Services Tax  (SST), green fee, caddy, buggy,   golf insurance, meals (lunch and dinner) and goodie bag.
  5. Each participant is required to declare his/her lowest current USGA/NHS Handicap.
  6. All play shall be in accordance with the Rules of Golf as approved by the Royal & Ancient Golf Club of St. Andrews, USGA and the Local Rules of the golf club approved by the Tournament Committee.
  7. The Bank reserves the right, provided that the Bank has obtained the winners consent to: -
    • Disclose details and particulars of the winners to third party for purposes of delivering the prize(s); and
    • Use, publish and display the names, photographs taken or other information of the winners for current and future advertising and publicity purposes without any compensation paid.
